MISSION

CHARM’s mission is to promote and advocate for healthy lifestyle choices aimed at addressing the leading causes of disease and death (cardiovascular disease-heart disease and stroke, cancer, HIV infection, diabetes, and obesity) and social ills (drug use, domestic violence, teen pregnancy, and unemployment) among rural minorities in an eleven-county service area of North Florida.

 

 

GOALS

 * Provide community outreach services to reduce the occurrence or prevent the leading causes of social ills, disease and death among rural minorities within the service area.

*  Refer clients to appropriate social and health care services within the service area as needed.

*  Provide advocacy for addressing the social ills and health care needs of rural minorities within the service area.
